For sky Standard or Sky plus boxes
1. Insert your sky viewing card into the sky viewing card slot located inside the pull down flap on the front of the sky box
2. Press the services button on your sky remote control
3. Select option 4 'system set up'
4. Select option 5 'system details'
All information required should now be displayed on your TV screen

For HD boxes
1. Insert your sky viewing card into the viewing card slot located inside the pull down flap on the front of your sky box
2. Press the services button on your remote control
3. Press zero followed by no.3 then select
All infromation required should now be displayed on your TV screen

PLEASE COPY THE ON SCREEN INFORMATION INTO THE MESSAGE BOX BELOW
ALONG WITH WHAT SKY PACKAGE YOU WOULD LIKE AND YOUR MONTHLY PAYMENT DETAILS (alternatley once the activation request has been sent you can call the office to give monthly payment details)
